Transition Planning

At Limerick Community Special School we believe time spent on Transition Planning will reap huge benefits for the child, family and staff in Limerick Community Special School.

After admissions is complete, many of the following planning for transition steps will be completed:

1. Reading of all reports and IEPs available

2. Parent/Guardian Meeting

3. Our Transition Pack leading to a Transition Plan

4. Visit to current setting

5. NEPS meeting/advice

6. CDNT (HSE) meeting/ advice

7. Social Story of starting in LCSS/ Key Visuals sent to parents

8. Intimate Care Plan/Medical Plan (as required)

9. Transition Padlet for Parents
